The controlled substances regulations require that people who are registered to use Schedule I and II substances should order them by way of the DEA form 222. The form must be filled carefully with a pen, indelible pencil or typewriter. The lines are numbered and only one item can be filled on each line. The number of lines completed must be duly recorded in the space provided at the bottom of the form. Name and address of the supplier must be entered before signing and dating the form by the registrant or another person authorized to do so.
